ID		Disapproved filings will not be re-opened. Corrections and additional information can be added to approved filings within 60 days of approval by requesting filing be re-opened. Cover letter required.
NE	Separate component header required for each form submitted. Side by side comparisons of all changes must be included. No letter required if information can be found in Transmittal Header
NV	No rule/rate filings accepted via SERFF Cover letter required.
NH	Separate component header required for each form submitted. No more than 15 component headers per filing. No more than 15 pages per attachment. Cover letter required Will not re-open closed filings.
NJ	Separate component header required for each form submitted. Side by side comparison of all form changes must be included. Final printed copies of approved materials must be sent to state via regular mail. Contact Mark Nussenfeld to have a closed filing re-opened. Cover letter required.
